Sounds like you ran into the same problem I just had.
Port for the hockey puck is in the wrong location, and is the wrong diameter, right?
3 options I can think of.
A. Use a catch can, and get rid of the OE crankcase breather stuff
B. Like I'm doing for now, just run the hockey puck to atmosphere and plug the turbo inlet port (things might get a little oily)
C. Adapt some radiator hose to go from the 'puck to the turbo inlet hose.

on my Forge hose, that port for the hockey puck up top is located on the outside of the bend in the lower right corner of that pic.
All other locations worked out alright. I ended up making a steel plug and let the crankcase vent to air.

i just got an email back from forge in orlando. they said that they dont make a tip hose for anything newer than 03. im so pissed. anyone ever installed one on anything newer than an 03? looks like ive got a red tip for sale. ****!
