Default Timing belt tensioner question

Since most of the time it is the tensioner that fails in a timing belt, does anyone know if it is the little wheel (A), the larger wheel (B), or the tensioner piston (C) that fails?

IIRC, it's the piston that fails. The seal on the piston shaft fails due to embriddlement (it gets kinda hot in there), lets the magic gas/oil out, and that's the story.
