can anyone tell me what can happen when someone replaces the tensioner on an a4 and doesnt do it correctly ?

What year and engine? If it is a rubber belt, then it can slip and throw off the timing, causing interference and engine failure. I don't know about tensioners on timing belts, or if there are any.
