Yeah, the A3 is pretty tough to heel/toe consistently- the pedal placement isn't that bad, but the brakes are touchy, with long travel; a firm brake makes things a lot easier, especially on the street where you aren't using max braking.

I'd first firm up the brake pedal. Get some Akebono front pads, and some Tyrolsport bushings for the front calipers. Give them a good bleeding with ATE blue or Motul fluid. Total cost is about $250 if you have somebody do it. This will help, though a lot of the A3's soft brake pedal is due to the ABS plumbing - can't do much about that. You can add stainless lines, but they're not much help in my experience.
See how you do with it then - if that's not improvement enough, then i'd be looking for a nice accelerator pedal (Third Foot, or something similar), or maybe modifying what you've got.
My A3 is DSG, so I can't really play with the pedals right now; My daughter's A3 is manual S Line, and I'll be visiting her over Christmas - check out possible mods when I'm there.

The brake pedal is too high. My wife's 328xi has the brake pedal even with the accelerator, which is much more comfortable and easier to move my big foot from accelerator to brake without twisting.

I'd have to disagree, but I have medium size feet. Part of the art of Heel/toe is fine tuning your technique and/or the car pedals to work best for you. I grew up with Porsches, and their floor mounted pedals are easiest for me to manipulate using my heel on the accelerator, and the ball of my foot on the brake. More modern German machinery like the Bimmer or Audi have suspended pedals, and are easier for me to work with a rocking motion ( left ball of my foot on the brake, right ball or side working the throttle), or even twisting my foot opposite the normal way and working the throttle with my toes. Whatever works smoothly is correct, and I've found very few cars I can't match revs on if i'm willing to adapt and experiment - though I've occasionally resorted to duct tape and wood blocks.
