Have you tried to make him understand all this with couple of friends, and 5-6 bottles of cheap vodka?? Always works. I had my share of dealing with a friend who was all messed up in a similar situation, and I lost it when he starting burning his hands with a cigarettes, so I gave him few good punches on stomach, slapped him hard, gave him a beating of his life and then spent entire night drinking with him... he was a changed person in the morning. He still thanks me for that day, sometimes.
