Default A6 3.0 quattro engine noise. Please Help

HI.
I purchased 2003 A6 3.0 quattro 5 months ago. it is 98000km now.
At that time engine noise was so loud, but the dealer said they gonna fix it before I take the car.
But when I got my car, nothing was not done. Not even cleaning.
They said come back later for check up, so I went to check up. But the mechanic said there is no problem at all.
It's been 5months and this week I'm going for another check up. And I really want to know the problems before I go to check up.
I have questions.
1) when I touch car engine I can feel strong vibration and really big noise. something like some kind of belt, i'm not sure.
also during driving car, when i press acceleration, rpm goes up and during this time engine makes big noise and when
rpm drops, engine noise reduce. this happens every time when i drive.
2) During driving the car, when rpm goes up and drops, I can feel little shock. I can't explain exact feeling but it's like pulling or
pushing car suddenly. I heard may be trasmission problem...
Also, when car is on D, the vibration to handle is much more than when it's on P or N.
3) My car is almost 100,000km , at this time is there a necessary maintance? Could you tell me what Audi offers for free at 100,000km.

Please, help me. I don't know anything about cars. I just think I bought a bad car and really disappointed with the service
that audi is offering. What should I say when I go to check up?
