I use 93 shell v-power everytime its the best and every 3 or 4 fill ups i put chevron techron in.

I use soybean oil and conola oil.
I also have a dual nozzle setup that sprays PAM directly into my intake manifold.

try revving to beyond 3.5k this doesnt mean the MAF but an air/vacuum leak

On a serious note though, Sams doesn't have 91 octane gas. - Not in any area I've been to. - Your car needs 91+ gas. - That could be your issue.
