Sounds like it's pretty low on fluid. Top it off and see if it helps. If it still does it looks like its time for a rebuild or new tranny.

I'd recommend doing a tranny flush. Do you live in colder weather? Because your car might just be cold. and not heated up so that will make it shift relatively hard.
