Default '03 A4 Fail Safe Mode

So, my Audi is in fail safe mode, but without any of the symptoms besides my tip mode won't work. I had an issue with the car before, but it wasn't throwing any codes about the tranny, just the coil packs. The car starts in first gear and goes up to fourth, not entirely sure, but I think the tranny isn't shifting to fifth gear. I do alot of around the town driving so I don't really use 5th gear that often. The tranny itself doesn't shift roughly going up in gears, but you can really feel it when it downshifts when I'm stopping at a light or slowing down. Possible theories and solutions?

I have more information. Upon further investigation I have concluded that the fail safe mode only kicks in when I shift to D. I have the sport mode option labeled S and fail safe doesn't kick in when I shift straight to that after starting the car. P, N, R, and S are normal, D brings up limp mode. Sometimes it'll kick back out of safe mode and Tip mode works just fine. D itself works fine and after paying close attention it does shift into 5th gear. Tip shifts into 5th as well, but it's rough, everything else tranny wise works just fine. Resetting the car itself (turning it off and then back on) brings up a normal gear selection display. Only after shifting into D does it go into limp mode. While not urgent, I'd like any opinions that the community may have. Also, is it possible to direct swap a manual tranny in place of an auto? I really hate automatic, it's boring and autos have more issues.

Run a scan on it. I'm sure there's a code in there.
